To apply for NHS Research Ethics Committee Approval please go to the applicants' section of the National Research Ethics Service website at:  http://www.nres.npsa.nhs.uk/

It is an electronic form. In order to complete the form, applicants will need to download the form-filler as well as the form from the NRES web site. If you need assistance downloading or completing the form please contact R&D.

Please ensure that you contact the R&D Directorate or your local REC for final confirmation as to whether you will need ethical approval for the study.
